Achieving Consultant status remains the ambition and pinnacle in the eyes of most hospital doctors. The stakes are high, the competition is intense, and the selection process is discerning. It is important, therefore, that the interview process is understood, and that the commonly asked questions and pitfalls are examined in order to prepare meticulously. Despite the importance of this interview, it is probable that most doctors do not invest sufficient time and effort preparing for it. All trainees now have to sit the nMRCGP examinations in order to qualify as a GP. The Applied Knowledge Test is one of the examinations that GP trainees need to do in order to obtain their nMRCGP. It is a summative assessment of knowledge based upon the RCGP curriculum. Candidates who pass this assessment will have demonstrated their competence in applying knowledge and interpreting information at a level which is suf?ciently high for independent practice. Candidates are able to sit the AKT at any point during their time in GP specialty training. Format of the test The test is a 3 hour long paper consisting of 200 questions. It is computer based and is taken at Pearson VUE professional testing centres, of which there are 150 around the UK. It is offered three times a year. The questions consist of single best answers, extended matching questions and algorithms, data interpretation, diagram, tables and picture questions. Approximately 80% of questions will be on clinical medicine, 10% on critical appraisal and evidence based clinical practice and 10% on health informatics and administrative issues. The single best answer questions often use a clinical scenario and only one answer is correct. The other options may be plausible and so they ask what is the SINGLE MOST appropriate option. The extended matching questions have a list of possible options for the theme and there will be several scenarios. The candidate needs to select the most appropriate option that best matches the scenario and each option can be used once, more than once or not at all. The picture/algorithm/data interpretation questions involve selecting the most appropriate option depending upon the question. 1102 candidates sat the AKT in April 2009. Their mean score was 143 out of 199 scored questions, with the best candidate gaining 183. The pass mark for this exam was set at 126 marks or 63.3%. This resulted in a pass rate of 83.8% for all those candidates taking the test. The mean scores by subject area were: ? 'Clinical medicine' 74% ? 'Evidence interpretation' 68.2% ?
