We would like to acknowledge all those who contributed to the monograph and hope that the context reflects both the similarities and difference of practice in this unique field of medicine. We hope that the reader finds this as a useful resource and is able to take away from the chapters, areas where we as a field need to continue to focus in order to move forward and improve outcomes.
