It has been known for more than a century that patients with renal failure develop bone deformities and osteoarticular complications leading to pain and frequently disabling consequences for physical activity; in addition, growth retardation is a frequent complication in children with advanced renal insufficiency. In recent years there have been major advances in the understanding and treatment of the complications described under the term "renal osteodystrophy". In recent years, our understanding of the underlying causes and mechanisms involved in these complications has made major progress. In addition, new drugs have been developed that will have a major impact in the prevention and treatment of renal osteodystrophy. The purpose of this book is to provide a state of the art overview of the various forms of renal osteodystrophy. Our aim is further to bring to the attention of adult and pediatric nephrologists, other specalists and practitioners the new developments in this area and to discuss their potential role in the treatment and prevention of the abnormalities of bone and mineral metabolism that occur in children and adults with renal failure.
PART I: GENERAL ASPECTS OF BONE TURNOVER AND GROWTH; 1. New aspects of bone biology (de Vernejoul, Marie); PART II: NORMAL PARATHYROID FUNCTION AND PTH ACTION; 2. Regulation of PTH secretion and synthesis and of parathyroid cell proliferation (Silver, Kilav, Sela-Brown, Naveh-Many); 3. Roles of parathyroid hormone-related peptide and the PTH/PTHrP receptor in skeletal development: implications for growth in children with end-stage renal disease (Juppner, Segre); PART III: PATHOGENESIS OF SECONDARY UREMIC HYPERPARATHYROIDISM; 4. Abnormal PTH synthesis, secretion and metabolism; peripheral tissue resistance to PTH; pathologic parathyroid parathyroid cell growth and apoptosis (Drueke, Rodriguez); 5. Parathyroid gland function in patients with renal failure (Goodman); PART IV; VITAMIN D METABOLISM AND ACTION IN CHRONIC RENAL FAILURE; 6. Vitamin D metabolism and action in chronic renal failure (Hsu, Young); PART V: PATHOGENETIC AND CLINICAL ASPECTS OF RENAL BONE DISEASE; 7. Cytokines and growth factors in renal bone disease (Martin, Gonzalez); 8. Osteitis fibrosa, osteomalacia and mixed bone lesions (Ramirez, Felsenfeld); 9. Adynamic bone disease in uraemia (Fournier, Moriniere, Said, Oprisiu, Tataru, Brazier, Marie, Cohen Solal); 10. Osteoporosis in uraemia (Fournier, Mazouz, Granos, Fardellone, Marie, Brazier, Cohen Solal); 11. Dialysis-associated amyloidosis (Bardin); 12. Bone diseases in children with chronic renal failure (Salusky, Kuizon); PART VI: SOFT TISSUE CALCIFICATION IN CHRONIC RENAL FAILURE; 13. Soft tissue calcification in chronic renal failure (Rostand, Thornley-Brown); PART VII: NON-INVASIVE AND INVASIVE DIAGNOSIS OF RENAL BONE DISEASE; 14. Classical serum biochemistry and new biochemical markers (Schmidt-Gayk, Roth, Bregulla, Reichel, Halleen, Ritz, Drueke); 15. X-ray, US, CT and MR imaging of skeleton and joints (Laredo, Bardin); 16. Imaging and intervention of parathyroid hyperplasia (Fukagawa, Kitaoka, Tominaga, Kuokawa); 17. The use of bone histomorphometry in renal osteodystrophy (Goodman); PART VIII: PROPHYLAXIS AND TREATMENT OF RENAL OSTEODYSTROPHY; 18. Medical therapy in predialysis chronic renal failure (Coen); 19. Medical therapy in dialysis patients (Ritz, Schomig, Odoni); 20. Management in renal transplant recipients (Cunningham); 21. New vitamin D analogues and calcimimetics (Coburn, Maung, Frazao, Elangovan); 22. Surgical management (Sarfati, Drueke)
