Part 1: Problem IdentificationIncludes forms and procedures to facilitate gathering of information to identify specific behaviors for change—a Child History Questionnaire, Problem Behavior Questionnaire, Task Analysis Questionnaire, and Pinpointing Form.
Part 2: Measurement and Functional Behavioral Assessment: Provides forms and procedures for behavioral assessment before, during, and after intervention—focusing on such techniques as School Antecedent-Behavior-Consequence (ABC) Recording, Interval Recording, Functional Behavioral Assessment, and Scatter Plot.
Part 3: Interventions: Behavior and Social Skills Intervention Plans: Skills training forms to address social, emotional, developmental, functional, and educational issues. Antecedent Interventions: “Coaching for change” procedures and forms helpful in adapting the environment to prevent problem behaviors—focusing on Relationship Enhancement, Frequent Feedback, Rule Establishment and Review, and other areas. Behavioral Interventions: Forms and procedures for teaching replacement behaviors, including Feelings Management, How to Solve a Problem, the 3-D Skills Approach, Social Scripting, and Cue and Review. Consequence Interventions: Designed to control positive and negative reinforcement, items include Behavior Contracts, a Daily Behavior Report Card, the Stop and Think Planning Essay, and School and Home Environment Learning Programs.

Dr Michael J. Asher, Ph.D., is a licensed psychologist in private practice at Behavior Therapy Associates, P.A. of Somerset, New Jersey, USA. He also is a consultant to numerous school districts and private schools in New Jersey and is a field supervisor for the Graduate School of Applied and Professional Psychology at Rutgers University. In addition, he conducts workshops throughout the United States and Canada on Attention Deficit Disorder and other disorders in children and adolescents. Dr Steven B. Gordon, Ph.D., ABPP, is a clinical psychologist and is on the faculty at the Graduate School of Applied and Professional Psychology, Rutgers University, USA. Dr. Gordon has extensive experience providing clinical services not only for children diagnosed with phobias, stress, selective mutism, obsessive compulsive disorders and depression, but also with adults coping with anxiety, depression and relationship difficulties. Dr Michael C. Selbst, Ph.D., is a licensed psychologist and a certified school psychologist. He has extensive experience working with pre-school aged children through adults, including individuals who are gifted, have learning disabilities, social- emotional and behavioral difficulties, and children with developmental delays, including those with autism and Asperger syndrome. Dr Mark Cooperberg, Ph.D., is a licensed psychologist. He has extensive experience conducting therapy and evaluations with children, adolescents, adults and their families, including individuals with a wide range of presenting problems. He has worked in numerous settings, including schools, specialized summer programs, community mental health centers, day treatment, residential placement, inpatient hospitals, and specialty outpatient clinics.
